چکیده

Sir, The newly-developed 'Thermal Sensibility Tester' , 1 ,2 has been used for several studies in the diagnosis of leprosy with interesting results, We discovered that it has not been clarified whether this Thermal Sensibility Tester was also designed for testing for early peripheral nerve damage (hand and foot) in leprosy patients, Some believe that this tester could be used in the fol low-up of nerve damage, In peripheral nerves unmyelinated C-fibres are supposedly also the earliest involved and therefore this belief would be an attractive concept. We have recently performed a study concerning the sensory thresholds in the hand in leprosy patients and have tested a control group of 50 normal people with the Thermal Sensibility Tester,3 We used 2 testers and before testing made sure that the tester was working, We tested 5 points on each hand 3 times and gave a positive score if 2 out of 3 were felt . This group was divided into 3 according to their profession; heavy, mixed and light work, Our findings are in Table I ,

Value of thermal sensibility testing in leprosy diagnosis in the field--field trial of a pocket device.

A handy thermal sensibility testing device has been developed and field tested in different centres in Africa and India. The device performed satisfactorily under field conditions and made testing for thermal sensibility in the field practicable and easy. Examination of the results of testing 260 persons, most of them having a few lesions of early leprosy, showed that the expected increase in t...

Early detection of neuropathy in leprosy: a comparison of five tests for field settings

BACKGROUND Early detection and treatment of neuropathy in leprosy is important to prevent disabilities. A recent study showed that the Nerve Conduction Studies (NCS) and Warm Detection Thresholds (WDT) tests can detect leprosy neuropathy the earliest. These two tests are not practical under field conditions, however, because they require climate-controlled rooms and highly trained staff and are...

Pure neural Leprosy: A case report

Leprosy is a chronic infectious disease endemic in Iran that mainly involves skin and peripheral nerves. Leprosy has several clinical forms. One of them is “Pure neural leprosy”. The diagnosis of this form is delayed due to lack of clinical signs in other tissues and can lead to permanent deformity. I will discuss my experience for 20 years in the diagnosis and treatment of this form of leprosy...

New insights and tools to combat leprosy nerve damage.

Nerve damage no doubt is the most feared complication in leprosy since it causes most of leprosy’s characteristic and stigmatizing disabilities. Paradoxically and sadly, however, its etiology and pathogenesis remain poorly understood, its clinical management difficult and its prevention far out of sight. Leprosy nerve damage particularly occurs during leprosy reversal reactions, which represent...

Early nerve damage in leprosy: an electrophysiological study of ulnar and median nerves in patients with and without clinical neural deficits.

BACKGROUND Peripheral nerve trunk involvement in leprosy is very common. However, by the time it becomes clinically manifest, the damage is quite advanced. If the preclinical nerve damage can be detected early, the deformities and disabilities can be prevented to a large extent. AIMS To assess the electrophysiological functions of the ulnar and median nerve trunks in cases of clinically manif...
